I present to your comments a case of a 55 y-o female patient (BMI 30,1) arrived at 4 PM to the ED because of severe respiratory distress of 3 days of evolution, with RR 60/min, SpO2 90%, perioral cyanosis, BP 106/77, HR 126/min, Tº 38,3 ºC. Co-morbidities were rheumatoid arthritis and hypothyroidism diagnosed only five months ago, under steroidal therapy since then. With a non-rebreather mask 15L/min, ABG showed paO2 90,2, paCO2 24, SaO2 97,8%. With the diagnosis of acquired community pneumonia, she was transferred to the ICU.
